If you choose to download, you may either print the form and complete it by hand or copy and paste the questions into an email and respond directly. Once finished, email your completed form as an attachment (subject line: Between Here & There) or include it in the body of your reply to our original welcome email.
Enrollment is confirmed only after the intake form has been received, reviewed, and approved. If you have any difficulty accessing or returning the form, please contact us as soon as possible so we can assist.
What happens next

Once your form has been received and approved, Lizzie will send a welcome email with next steps, your Thrive membership access details, and instructions for booking your 30-minute intake Zoom session.
If we determine the cohort is not the right fit, your payment will be fully refunded.

1. Nature of the Program (Disclaimer)
This cohort is a coaching and personal development experience. It is not psychotherapy, medical care, mental health treatment, legal advice, or financial advice.
If you are experiencing a mental health crisis, emotional instability, or require clinical therapeutic support, this program is not a substitute for appropriate professional care. We reserve the right to decline enrollment if we believe additional clinical support is necessary.
2. Enrollment & Confirmation
Enrollment is a two-step process:
Full payment is received.
The required intake form is submitted within 48 hours of purchase.
Enrollment is confirmed only after the intake form is reviewed and approved.
If the intake form is not submitted within 48 hours, payment will be refunded and the seat released to the waitlist.
If, after review, we determine the cohort is not an appropriate fit, a full refund will be issued.
Cohort size is limited to a maximum of eight (8) participants per group.
3. Refund Policy
Due to the limited and intimate nature of this cohort experience, all sales are final after the stated refund window.
Participants may request a full refund within 48 hours of purchase, provided that:
• The cohort has not yet begun, and
• No program materials, recordings, or bonus resources have been accessed.After 48 hours from the time of purchase, all payments are non-refundable and non-transferable
After the program start date, no refunds will be issued for schedule conflicts, missed sessions, voluntary withdrawal, or personal circumstances. If a participant is removed for suitability or safety reasons unrelated to misconduct, a pro-rated refund may be issued at our discretion based on remaining sessions.
If a participant is removed due to misconduct, breach of confidentiality, or disruptive behavior, no refund will be issued.
4. Transfers
A one-time transfer to the next available cohort may be requested up to fourteen (14) days before the program start date.
Within fourteen (14) days of the start date, transfers are permitted only if the seat can be filled from the waitlist.
Transfers are not guaranteed and are granted at our discretion.
5. Participation, Conduct & Confidentiality
To maintain a safe and respectful container, participants agree to:
Maintain confidentiality of all personal information shared within the cohort.
Engage respectfully with the facilitator and other participants.
Participate in good faith without harmful, disruptive, or inappropriate behavior.
We reserve the right to remove any participant whose conduct materially disrupts the group, violates confidentiality, or compromises the safety of the container.
All removal decisions are made to protect the integrity and well-being of the group.
6. Attendance & Missed Sessions
Live participation is expected. Sessions are not recorded.
Missed sessions, late arrival, or early departure do not qualify for refunds, credits, or transfers.
7. Cooling-Off Compliance
Where required by applicable consumer protection laws, participants may cancel within the statutory cooling-off period following purchase for a full refund, provided the program has not yet begun.
8. Limitation of Liability
By participating in this cohort, you acknowledge that personal growth work may involve emotional discomfort and self-reflection. You voluntarily assume responsibility for your participation.
To the fullest extent permitted by law, total liability for any claim arising out of participation in this program shall not exceed the amount paid for the cohort.

We welcome online intake form submissions. The questions on the online form below are identical to the downloadable form you received in your email and linked above. We recommend that you print or review the questions in the downloadable form to gather your answers before using the online form. The online form must be completed in one sitting. Answers are not saved, and you will need to rewrite them if the form is not completed all at once or if your internet connection fails.
